Duties and
Responsibilities:
Comfort Home Care is seeking a Massachusetts licensed Registered Nurse that can:
Provides initial and ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn.
Initiates the plan of care, necessary revisions, and performs OASIS assessments at appropriate time points when applicable.
Initiates and performs appropriate nursing interventions.
Coordinates services, informs the physician and other personnel of changes in the client's condition and needs.
Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
